Vaccines for Students
Vaccines for Student Program:
Pathways Wellness Program offers onsite immunization clinics to help students receive the vaccines they need to attend school. Parents and Family members are welcome to join!
For those who cannot make the onsite clinic at school, they are welcome to the Weekly Wellness Clinic in our South Hills Office held every Wednesday.
PRE-CLINIC
- Pathways will work with the school to confirm the date, time and location for the clinic and provide communication materials to assist in sharing the details to students, parents and staff.
- Families are encouraged to register through the provided link where they can make an appointment for themselves and/or their child/children. Paper letters with a QR Code and a Paper Registration Form are also available.
- Pathways will verify insurance coverage and coordinate a VFC provider for those students who have Medical Assistance.
- Parents will review and sign a Consent form for their child to receive the requested vaccines.
DAY OF CLINIC
- Pathways staff of credentialed Immunization Clinicians and Clinic Administrators will bring all vaccines and supplies needed for the clinic.
- Patients will receive a Notification of Vaccination to provide to their doctor and/or school nurse
- Pathways will remove all medical waste from the site
POST-CLINIC
- Pathways will send a post-clinic survey to collect comments and recommendations
- All vaccines will be reported in the state database (PIERS) to ensure communication back to their medical home.
