Health Coaching Packages
Our Health Coaching programs are designed to personalize your employees’ journey towards health and wellness. Some individuals need a more comprehensive approach and coaching to tackle specific hurdles. Health Coaches analyze employees’ current state of health and help them create realistic goals. Then they make a plan to achieve it and include regular points of contact to keep employees focused on goal achievement.
Base Package: Introduction to Health Coaching
60 minutes session
Includes a 60 minute one-on-one session with a Health Coach. The following topics will be reviewed: Health Risk Assessment & biometric screening results review, addressing client’s health concerns and behavior changes.
Discover Your Journey Health Coaching
(3) 60 minutes sessions + email correspondence
Session 1: Initial coaching session covers Health Risk Assessment, food journaling, goal setting, meal prep planning, and weekly activity goals.
Session 2: The Health Coach will help set goals and to-do’s for the week ahead.
Session 3: Define long-term goals and determine the need for additional sessions.
Make It A Habit Healthy Coaching
(5) 60 minutes sessions + email correspondence
Session 1: Initial coaching session covering Health Risk Assessment, food journaling, goal setting, meal prep planning, and weekly activity goals.
Session 2: Setting goals and/or homework for the week ahead.
Session 3: Review goals that are being reached and any areas that may need changed.
Session 4: Setting goals and/or homework for the week ahead.
Session 5: Review the last six weeks’ achieved goals, any obstacles, and setting of future short and long-term goals.

Our Health Coaches are high-leveled and experienced professionals who are trained in health coaching techniques and are empathetic to the needs of employees. Each health coach has a minimum of a bachelor’s degree and at least 3 years of experience.

How often can I contact my coach?
As often as needed. Some people require only once a month while others will be more often. You may schedule appointments whenever needed. We want your relationship to be of benefit while navigating lifestyle changes.
Is my personal information confidential?
Our respect for your privacy and confidentiality is paramount. Only your Pathways to SmartCare coach has access to your information and will never share that information with your employer, co-workers or other companies who might be involved in your wellness journey.
